There are 5: Elevation, Striking, Origin, Attack, Chaos
For the first 4:
For 80 types, old (pre-revamp) rare grade dungeon boxes which do not need key.
For 90 types, old (pre-revamp) epic and unique grade dungeon boxes which do not need key.
For 93 types, I mostly get them at the end of Rune 8 man (either norm or HC not particularly sure) raids.
For 95 types, new (revamped) unique grade dungeon boxes which require 30 key to open.
For Chaos plates (95 only now)
Wonderful Theme Park exchange NPC for 1250 points. You can get 2.6k points max a day by doing the Wonderful Theme Park daily quest.
